STANDARD REQUIRED FORMS TO BE RETURNED - CLASSIC COURSES

Medical Record - Adult

This form does NOT require a physician's visit or signature upon submission. Make sure to complete ALL sections. Don't forget: height, weight, blood pressure and signatures. If you have health insurance, include a copy of the front/back of the card.

Financial Commitment Booklet

Complete and return with other forms.

Participant Acknowledgement and Assumption of Risks and Liability Release and Indemnity Agreement

This form must be returned unaltered with required signature(s) and dated. Do not forget to include the applicant's date of birth. It cannot be digitally signed.

Course Survey Consent Form

The information we gather from this survey will aid us in assessing the educational value of your experience.

ADDITIONAL REQUIRED FORMS TO BE RETURNED - CLASSIC COURSES

ADHD Medication Questionnaire

Does NOT require a physician's visit or signature, but is required if you are prescribed an ADHD medication.

Asthma Medication Questionnaire

Does NOT require a physician's visit or signature, but is required if you have Asthma.

Counseling Questionnaire

This questionnaire MUST be completed by your therapist and is required if you are currently in counseling or have been within the past two years. It is your responsibility to confirm the form arrives by your due date.

Diabetes Management Plan

This form MUST be completed by your physician and is required if you have diabetes.

Dietary Allergen Questionnaire

Does NOT require a physician's visit or signature, but is required if you have food allergies.

Physical Exam Form

This examination form MUST be completed by your physician if you are participating in an international course or a course with an international component, or if required based on a current medical condition.

Psychotropic Medication Questionnaire

This questionnaire MUST be completed by your prescribing physician and is required if you are taking any medication capable of affecting your mind, emotions or behavior.

Scholarship Application

If you are applying for scholarship, we will not evaluate your request for support until we receive and approve all required paperwork. Any delay in meeting your due dates will be interpreted as lack of motivation and will affect your award status.

Orthopedic Questionnaire

Does NOT require a physician's visit or signature, but is required if you have any orthopedic issue.

Asthma Action Plan

This questionnaire MUST be completed by your physician and is required if you take a daily asthma medication that contains a steroid. If you are delivering the form to your physician, it is your responsibility to confirm the form arrives by your due date.

Request to Attend With a Friend/Relative

If you are applying for a course with a friend or family member, this form must be completed. It will be reviewed when ALL required paperwork has been received and a phone interview has been completed.

Request to Override Minimum Age Requirement

Required if you are applying for a course outside of your age group.
