This is your course web page. Bookmark this web page for future reference. It is your “go to” location for all forms and information specific to your course. Refer to your Registration E-mail to determine which forms are required for you or your child’s application.
All forms (with the exception of the Participant Acknowledgement and Assumption of Risks and Liability Release and Indemnity Agreement) can be completed digitally. However, you must print out the liability form mentioned here, sign as required, and fax, scan, or mail back to us.

Application Deadline Policy: All applicants must adhere to the due dates listed in their Registration E-mail for payments and forms. If deadlines are not met, applications will be moved to INACTIVE status (Reference the Policy Booklet, page 1).

You need the most current edition of Adobe Reader in order to access and complete these forms. You can download Adobe for free.
How to Complete Your Forms
Option 1: Using Adobe, type your responses directly into the fields provided. You can choose to print these forms and sign with blue or black ink and return OR you can use an electronic signature. See the Instructions for Creating an Electronic Signature. (EXCEPTION: Participant Acknowledgement and Assumption of Risks and Liability Release and Indemnity Agreement)
Option 2: Print each form and complete with blue or black ink. Applicant and Parent/Guardian (if applicable) MUST sign where indicated.
(Mac USERS: Use Adobe. Do not use Preview, the Mac Reader software)
How to Submit Your Forms

Option 2: SCAN as .pdf documents (no larger than 10mb per file); E-MAIL to studentservices@ncobs.org. Include applicant name in subject line of e-mail
